It is 9 p.m. Your child had another hard day, and you are sitting with your phone, trying to figure out who to call. Getting mental health care for a child in Riverside and San Bernardino counties can feel like standing in a long hallway with many doors and no signs on any of them.

This guide puts the signs up. It explains which door to try first, what each kind of care means, and what to ask when someone answers. It ends with a list of phone numbers and programs for children and teens. Every listing was checked against official websites on September 16, 2026.

Key takeaways

  • If your child is in danger right now, call 911. For a mental health crisis, call or text 988 any time.
  • Both counties send crisis teams to help children. Riverside: 951-686-4357. San Bernardino: 800-398-0018.
  • If your child has IEHP, you can call IEHP for therapy without a doctor’s referral.
  • The county handles more serious needs, like crisis care, hospital stays, and drug or alcohol treatment.
  • With Medi-Cal, there is “no wrong door.” Your child can start care through IEHP or the county.
  • No insurance does not mean no help. Both county systems say they do not turn people away because they cannot pay.
  • Programs change. Always call before you drive somewhere.

If your child is in danger right now

Call 911 if your child is hurt, has taken something, or could hurt someone right now. You can also go to the nearest emergency room.

For a mental health crisis, you have more options:

  • 988 Suicide & Crisis Lifeline. Call or text 988, or chat at 988lifeline.org. It is free, private, and open 24/7.1
  • Riverside County. Call 951-686-4357 (951-686-HELP), any time. It is free, and bilingual counselors are available. This same number sends a mobile crisis team to help people of all ages.2, 3, 4
  • San Bernardino County. Call 800-398-0018 or text 909-420-0560, any time. The Community Crisis Response Team helps children and adults in English and Spanish.5

Some walk-in crisis centers see children and teens, and some see only adults. The list below shows which is which.

Four doors for a child’s mental health care in the Inland Empire: danger, crisis, everyday therapy through IEHP, and serious needs through the county. To text San Bernardino County’s crisis team, use 909-420-0560. Tap the image to read it full size.

Step 1: Find the right door

With Medi-Cal in these two counties, mental health care comes through two systems.

IEHP handles everyday mental health care. This includes therapy for things like anxiety, sadness, or behavior problems at home or school. IEHP says members can see a behavioral health specialist for therapy without a referral.6 Call 800-440-4347 (TTY 800-718-4347). The IEHP behavioral health team answers Monday to Friday, 8 a.m. to 5 p.m.6, 7 Ask for a therapist who works with children or teens.

IEHP can also give free rides to mental health appointments for Medi-Cal members. Ask at least 5 days before a routine visit.8

IEHP is not the only Medi-Cal plan here, and the plan lineup changes over time. Your child’s Medi-Cal card names the plan. Call the member services number printed on that card and ask for behavioral health; if the card names IEHP, use the number above. The county door below is the same for every plan.

The county handles more serious needs. This includes crisis care, hospital stays, intensive care coordination, intensive services in the home, and therapeutic foster care.9 Drug and alcohol treatment for Medi-Cal members also goes through the county.10, 11

  • Riverside County CARES Line: 800-499-3008, open 24/7. The county serves children and teens up to age 17 and can check mental health or substance use needs.12
  • San Bernardino County Access Unit: 888-743-1478 (TTY 711), open 24/7. They help families find the right provider.13

There is no wrong door. California’s “No Wrong Door” policy says children and youth under 21 should get timely care no matter which system they call first. A child can even get care from IEHP and the county at the same time.14 If you get bounced between the two, say “No Wrong Door” and ask them to connect with each other while you stay on the phone.

Step 2: Know the levels of care

Programs use short names that can be confusing. Here is what they mean.

  • Outpatient therapy. Your child meets with a therapist, usually once a week or so, and goes home.
  • Intensive outpatient program (IOP). Several hours of therapy a few days a week. Many run after school.15, 16
  • Partial hospitalization program (PHP), or day program. Treatment for most of the day on most weekdays. Your child sleeps at home.17
  • Crisis walk-in center or crisis stabilization unit. A short stay, less than a day, to calm a crisis and plan next steps.18
  • Inpatient care. Your child stays in a hospital overnight to stay safe and get treatment. Emergency rooms and crisis teams usually decide if this is needed.

A child can move between levels. A day program might step down to an after-school program, then to weekly therapy.

Step 3: Ask these questions when you call

Write the answers down. Keep this list on your phone.

  • Do you see children my child’s age at this location?
  • Is this program open right now, and how long is the wait?
  • Do you take my child’s exact IEHP plan, or do we need county approval first?
  • Does my child need a referral? Who starts it?
  • What does it cost if we have no insurance? Do you have a sliding fee?
  • What papers do I need to bring?
  • Does the therapist speak our language, or will there be an interpreter?
  • What number do I call after hours?

One more tip: a program’s website may say it “works with IEHP.” That does not mean IEHP has approved that program for your child. Call IEHP to check.

If you have no insurance or very little money

Start with your county line. San Bernardino County’s behavioral health department says no client or potential client “will be denied access to services due to inability to pay.”19 It uses a sliding scale based on family size, income, and assets.20, 19 Riverside County says “no one is denied care based on their ability to pay.”12

Community health centers are the next stop. Many offer counseling for children with fees based on income. Several say they never turn anyone away because they cannot pay. University training clinics also offer low-cost therapy, with trainees supervised by licensed clinicians.21

Ask every clinic whether your child may qualify for Medi-Cal and how to apply.

Frequently asked questions

Do I need a referral from my child’s doctor to get therapy through IEHP?

No. IEHP says members can see a behavioral health specialist for therapy without a referral. Call 800-440-4347.6

What if IEHP and the county each say the other one is responsible?

California’s No Wrong Door policy says children and youth under 21 should get timely care no matter where they start. Your child can get care from both systems at the same time. Ask the person on the phone to connect with the other system while you wait.14

My child has no insurance. Can they still get help?

Yes. Both county systems say they do not deny care because a family cannot pay.19, 12 Community health centers in this guide offer sliding fees. Ask each clinic whether your child may qualify for Medi-Cal.

Can I take my child to a crisis walk-in center instead of the emergency room?

For a mental health crisis, some walk-in centers see children. Check the ages first. Riverside’s County Farm Road urgent care serves adults only.28 If your child is hurt, has swallowed something, or has a medical emergency, call 911 or go to the ER.

What is the difference between IOP and PHP?

An intensive outpatient program (IOP) meets for a few hours on a few days each week, often after school. A partial hospitalization program (PHP) runs most of the day on most weekdays. In both, your child sleeps at home.17, 15

How current is this list?

Every listing was checked against an official source on September 16, 2026. Programs open, close, move, and change ages. Call before you go.
