Refer a patient
Referrals go to Inland Psychiatric Medical Group (IPMG), not through this website. Please keep patient details out of the forms and email on this site.
To refer, use IPMG's referral form. It asks for the patient's name, date of birth, phone and address, your office's phone and fax, and a referral letter or authorization.
Your patient doesn't need a referral to start care. They can call (909) 707-6261 or request a callback themselves.
One important note: this site doesn't handle appointments or health questions — IPMG does, by phone — and email isn't a secure or private channel, so please keep health details out of it.
